The bnx2 firmware is not loaded from initrd on bootup.
The following message is logged:

I've confirmed that the correct initrd is being used.
I've extracted the files from the initrd and the correct firmware is present:
/lib/firmware/bnx2-06-4.0.5.fw
If I rmmod and modprobe the bnx2 module after the system has finished booting it is able to load the firmware correctly.
