Information provided here is from -13 since -14 fails to boot on this host. Unfortunately this is a colocated box so I do not have access to the console on boot to troubleshoot the issue.
Unfortunately -13 is unusable too due to the netback uevent bug which prevents domUs from allocating virtual interfaces, as you can see from the kernel log.
                
-12 is the last usable version for me. I am not sure if there is a way to debug boot issues without console access.
