Bug#556294: GPLv2-licenced code using OpenSSL

usr/iscsi/chap.c uses SHA1 and MD5 functions from OpenSSL.  The GPLv2
and OpenSSL licences are incompatible, so these need to be replaced
with an alternate implementation.
