automatically on boot.  Once manually loaded it works fine.

With assitance from Moritz Muehlenhoff and Bob Copeland (via the
ath5k-devel list), I realised the problem was my fault.  The machine had
previously been an etch machine and had had the MadWifi drivers from
ethc built and installed, these created /etc/modprobe.d/madwifi which
blacklisted ath5k, causing the problem.
