no, it has only shown up until now on 1 box,
we have no other report that root on lvm does not work.
this very likely local breakage.

you don't provide any info to look at,
so i'll ask you for several output:
cat /etc/fstab
cat /proc/cmdline
lsmod
ldd /sbin/vgchange 
sh -x mkinitramfs -o /tmp/test.initramfs

aboves should give enough infos, what's going wrong.

so it should also be on the initramfs as it is installed.
doublechecked it gets included.
