Say I build a kernel with /proc/config.gz. 2 days later I notice that
I forgot some module and just build that.

If the config.gz is compiled into the vmlinuz then that won't refelct
the actual kernel+modules anymore.

If there is a config.ko that could get rebuild on every change to
.config and thus include the update to build an extra module.

I kind of like that.

For distributions I think it is rather irelevant. They build the
kernel debs or rpms and that is that. They don't change during use. If
the .config changes a complete new deb/rpm is build.
