Both 1 and 2 seems to be the same as bug 348147
(see http://bugs.debian.org/cgi-bin/bugreport.cgi?bug=348147;msg=48)

3) It should be documented that hooks and scripts may not contain
   hyphens in their names.

This looks like bug 344639 which should be fixed already.
