I'm encountering the same problem, I think.
I've got LVM running on top of RAID-1. When I get dropped to a busybox
prompt, USB isn't loaded yet (could we do this _before_ messing with the
root FS, please?) so I can't poke around. But it looks like dm-mod was
loaded and none of the md bits were. So I would guess that something is
wrong with the prereqs mechanism... maybe.
