Agreed, this kind of failure almost always indicates that
hardware is failing, and once it starts failing it usually
fails at an accellerated rate until you can't use the
drive any more.

This problem may not have shown up in 2.6.12 because
at that time your hardware was not exhibiting the problem.
It also may not have shown up because the code may have
been changed a bit and is now more sensitive.

But in any case I think the chance that is a disk problem
is overwhelmingly more likely than a kernel bug.
