At least as far as d-i is concerned (AFAICT), you have to put LVM on top of an existing partition table; you can't just use the full /dev/sda or whatever. (The command-line lets you get around this).
However, even if you do use the command line to get around d-i limitations, its still not safe to have /boot on LVM (at least according to all the documentation I've seen).
I believe the only way to use d-i on machines with only > 2TiB disks is to rebuild the kernel to enable EFI GPT.
If EFI breaks iPods, could we maybe have an 'enable_efi' kernel command-line option to enable the support? That way, everyone could be happy.
