I remember that, when using the Woody KDE3 backport, some files (namely /usr/share/doc/kde/HTML/en/common/*) would misteriously disappear from the filesystem even though the package was installed. I never was able to find out the cause, but it did happen. So, perhaps he should check if /usr/share/icons/crystalsvg/16x16/filesystems/folder_inbox.png is actually in his filesystem. Another idea would be to create a new system user and start kde and see if the problem persists (should the problem then be narrowed down to ~/.kde, for example).
