I have done this is a number of places, including a few ISP's in the
UK; it's not really difficult, however like you said, undocumented.

I'm willing to write a HOWTO, I have been meaning to write one about
using MySQL as an authentication and configuration database (apache
vhosts, user accounts, mail, and other things) for months now anyway,
with different methods.

Exim has all the support needed built in both pop-before-smtp, and
MySQL authentication. I have not tried with any other SMTP server
however. Does anyone know of any patches or daemons that support it?

POP3 however, is a different matter: I could not find a decent pop3
server that supports it, though have recently been testing tpop3d,
for use in a live environment, and certainly looks interesting, may
have more to comment in a few days when I've finished browsing
through its code.

I haven't ever had the need to use IMAP, though I am willing to do
the research, if anyone has any information, it would be gratefully
received.

I'll read through the HOWTO-HOWTO tonight (been meaning to for months
now), and do some research into it, then start on it - I've got a
couple days spare.
