That did the trick. I was using Clearlooks. When I opened the theme
selector, "Custom Theme" was selected (well, it was always "Custom
Theme" selected when launching the theme selector before as far as I
remember, no matter what theme I did actually use). Upon selecting
Clearlooks again, the mixer applet appeared on the panel. And now the
theme selector seems to remember the correct theme as well. And two
icons that were broken ever since I upgraded to gnome-2.12 also appeared
correctly in the menu.

Btw. unison now shows the following difference in the gconf database
(diff: -broken, +fixed):
