What command are you running exactly?
gnatmake -o test_indexed_sequential_io test_indexed_sequential_io.adb

I've got that command sitting in a terminal window right now, thus:
charles@mandala1:~/projects/Ada/db1/ioseq/src/xmit$ gnatmake -o test_indexed_sequential_io test_indexed_sequential_io.adb
gcc-4.1 -c test_indexed_sequential_io.adb
