This unfortunately puts us into shot-in-the-dark territory.  However,
Jim pointed out one problem (lack of REG_INC notes in all cases) and
I found another (probably more serious; fixup insns generated for
unadjustible POST_MODIFY were being emitted in the wrong place).  So,
if you wouldn't mind trying the appended revision (changes only
ia64.c)?
