Yes, you are mixing up things here.

The type of malware you are talking about needs to be run on a host to
become a problem. As long as it doesn't get executed, you don't have a
problem. If it gets executed, then you have to consider the box
compromised, in which case doing a clean install is most likely the only
way to restore a known good state. Firewalls don't help here.
