I use D-Link DFE-570TX quad cards.

Much cheaper than all the rest. Works great. tulip driver.

02:04.0 Ethernet controller: Digital Equipment Corporation DECchip 21142/43 (rev 41)
Subsystem: D-Link System Inc: Unknown device 1112
Flags: bus master, medium devsel, latency 32, IRQ 12
	I/O ports at d000 [size=128]
	Memory at dc003000 (32-bit, non-prefetchable) [size=1K]
Expansion ROM at d8000000 [disabled] [size=256K]

3 quad cards in one box. No problems.
