I have exactly the same problem on a Thinkpad T440p and a firmware
update didn't help. I "solved" it by installing grub as
BOOT/BOOTX64.EFI, too:
| # find /boot/efi/EFI/
| /boot/efi/EFI/
| /boot/efi/EFI/debian
| /boot/efi/EFI/debian/grubx64.efi
| /boot/efi/EFI/BOOT
| /boot/efi/EFI/BOOT/BOOTX64.EFI
The bug is that grub-install does not check the return code of
efibootmgr.
