It isn't as simple as making a transitional package. If there are
customisations then they will need to be ported over. Upgrade instructions
are in the request-tracker5 package, and the description notes that it can
be installed simultaneously with request-tracker4.

But we could add
something like "Instructions on how to upgrade are in the request-tracker5
package, which can be installed simultaneously."

cool. i would read "simultaneously" as meaning that people should upgrade to trixie, but keep the release-tracker4 from bookworm installed, install r-t5, do the conversion and then remove r-t4, as opposed to doing the conversion in bookworm
