Hey,

A quick summary of the problem:

Alan is using '02:04.0 Network controller: Intel Corporation
PRO/Wireless 2200BG [Calexico2] Network Connection (rev 05)' with
ipw2200 driver and correctly loaded non-free firmware blob
'ipw2200-bss.fw'. After he disables the wifi radio via hardware button
or via network manager he is unable to re-enable it. The only way to fix
it is to reset BIOS settings to default configuration (after such a
reset wifi is automatically back on). Rfkill is also unable to re-enable
wifi but after using 'unblock' option and resetting BIOS the wifi is
still disabled (but can be re-enabled by normal means - once).

The only suspicious dmesg message is 'ipw2200: Failed to send
CARD_DISABLE: Command timed out.'
