Are refering to
13.7 Changelog files
The Debian changelog file (debian/changelog) should explain briefly what
modifications were made in the Debian version of the package compared to
the upstream one. Other changes and updates to the package should also
be documented in this file.

I don't see that this differs substantially in meaning from 5.3. It still
unclearly implies that upstream-only changes should not be mentioned in the
Debian changelog.
Also note this from 13.7:
The format of the debian/changelog file is described in
debian/changelog, Section 5.3.
This could be taken as incorporating 5.3 by reference.
