Install the sources of the hello package and look at changelog, that's what
i did.  Apparently the format is

<package> (<version>) <distribution>;urgency=<urgency>

  * <changelog entry>
  * <another changelog entry>

 -- <maintainer>  <date>

with <..> to be replaced by the proper values.
