Bug#2669: sgml2txt: <appendix> broken

The sgml2txt script doesn't handle appendices correctly.

The section headings in the appendices are letters instead of numbers;
this is probably correct.  However, the counter isn't reset to
0/1/whatever, so if the last chapter was 19, the first appendix is T.

When it comes to the table of contents, the appendices are not letters,
nor is the counter reset (which is at least consistent, I suppose ...);
so in my little example, the first appendix would appear as 20.

I've tested this with the SCSI-Programming-HOWTO as well as one of my own
documents.

This seems to be an upstream bug, or could even be a bug in groff (I have
version 1.09-3).
