* I broke accounting in 2001.10.09, fixed now. This also fixes a problem
with utmp writing. It seems that if do_acct is set to false then the utmp
record will have the IP address from pslave.conf even if that IP address
was over-ruled by the RADIUS server!

* Fixed utmpfrom in the default config documentation. Also made the variable
in the source consistant with the config file so I don't make that mistake
again.

* Documented the environment variables used by portslave.
