Bug#916881: med-fichier seems to have started a transition without package rename

Dear maintainers,

With a recent upload of med-fichier the autopkgtest of gmsh fails in
testing when that autopkgtest is run with the binary packages of
med-fichier from unstable. It passes when run with only packages from
testing. In tabular form:
                       pass            fail
med-fichier            from testing    4.0.0+repack-1
gmsh                   from testing    3.0.6+dfsg1-4
all others             from testing    from testing

I copied some of the output at the bottom of this report. Reading the
error and looking at the content of the libmed1v5 package it seems that
a transition was started without renaming the package:
/usr/lib/x86_64-linux-gnu/libmed.so.11.

Currently this regression is contributing to the delay of the migration
of med-fichier to testing [1].
