Comments/Problems:

I tried to install using the method described here: https://www.debian.org/releases/stable/i386/ch04s03.en.html#usb-copy-flexible (At first, I tried to add the installer to an existing USB stick with existing syslinux, but upon failure, I procured an empty (8GB) USB stick and followed the installation instructions word-for-word). The only difference I can think of compared to the above instructions is that I created a new partition table with one 0C type partition.

The "CD" is not detected, I even if I specify the USB drive or partition manually, or mount the USB partition manually and point to the ISO file manually as the "device file for accessing the CD-ROM".

I tried both on bare metal on the intended system, and in kvm on Debian Stretch and Ubuntu 18.04. I tried the i386 and multiarch installers, but the amd64 version may also be affected.

I can also upload a USB stick image (about 400MB compressed) to a server for someone to download, if you cannot reproduce the error.

As a corollary, I must note, that even if I use the simple method of "cp debian.iso /dev/sdX" to set up the USB stick, it does not automatically recognise the CDROM: I must specify the /dev/sdX that refers to the USB drive manually (but it does work after that). I only tried this on kvm.
