The (error) messages that you get,
boil down to "You can not copy a file _into_ a single file."

Yes. It is safe to assume that. After all the Internet is large.

So the question "Where is it documented?" remains.

Do a web search on "How to modify a ISO 9660 image"

If it returns something that should go in the installation manual,
let this mailinglist known.
