Bug#232415: mkinitrd fails on cciss drive hardware

It might be worthwhile to take a look at this and at bug #235617 in the
same pass - both deal with the device not existing for controllers which
use non-standard devices (or rather, non-SCSI/IDE devices). For the
cciss and DAC960 drivers, the devices are normally of the form
/dev/(cciss|rd)/cXdYpZ where X, Y, and Z are the controller number, disk
number, and partition number, respectively. In both cases it devices of
the form /dev/(cciss|rd)/discX/partY were attempted to be used.
