It looks like the 3c59x nic driver isn't included on the floppy images
from

http://people.debian.org/~joeyh/d-i/images/daily/floppy/access

dated on 08-Oct-2004

I checked out the source code for debian-installer from svn and noticed the
nic-modules udeb was removed from the root image to make room for
brltty.  It appears the nic-modules weren't added to the net-drivers
image.  When I added the line

nic-modules-${kernel:Version}

to the

installer/build/pkg-lists/access-net-drivers/i386.cfg

file, I could build images with the 3c59x driver available during the
install.
