I tried d-i Beta 2 netinst one more time today, and this time the b44 driver worked as it should. To be consistent, I tried kernel-image-2.4.23 and kernel-image-2.4.24 as well. All worked.

Since the d-i kernel and kernel-image-2.4.23 did not work last night, while Broadcom's bcm4400 driver did, I can only conclude that the b44 in those two kernels is somewhat flaky. I'm not sure if the 2.4.24 b44 driver has improved or not, or if it would have worked when the others didn't.
