Grisu gave some URLs, e.g. you can browse
  http://ddtp.debian.org/debconf/template_unstable/
and look for templates-de.po files within package directories.

Joey Hess wrote a script to get translated templates from the ddts, see

AFAICT it works this way, as does also
  http://www.debian.org/intl/l10n/po-debconf/de

Petter Reinholdtsen wrote scripts/gettext-helper, I did not yet use it,
but it is definitely worth a try.  This script lets you manage all
debian-installer PO files (in CVS) within a single PO file.

As you can see, there are many different solutions; you can choose your
preferred one, but things are simpler if all translators for a given
language agree on a unique scheme.
