Bad CD -- or bad cd device, or bad kernel driver.  At any rate,
fundamentally a hardware issue.

This is bad.  Do you get this using woody as well as Potato?  If so,
then it's a fixable problem, and we need a grave bug opened.

Hmm?  There's also network/TFTP install which actually should work
pretty well on a NewWorld PowerMac.  See the install manual.

Note that we are only accepting problem reports against the 3.0/woody
version.
