Are you using 'dd' to create the floppies?  RAWRITE seems to have some
known problems.  If you have to write from DOS, you might have better
luck with RAWRITE3 (I've been told), or you could use the "files in
DOS partition, floppyless install" option.

Other causes:

 - bad floppy

 - need to use the  floppy=thinkpad  bootarg
