As far as we know, that's only happened once in the two year development period of the nslu2-linux project (it was a bug I introduced in a very early version of the slugimage tool).

However a bug in any of the tools in the flashing chain that
would cause wrong flash blocks getting overwritten could indeed
brick it, so some carefulness is a good idea..
