Bug#771679: #771679 apache2-mpm-itk fails to restart properly on some occasions



This looks like a long standing bug going back at least as far as Lenny. I'm
still running a Lenny server with apache2-mpm-itk which exhibits similar
symptoms.

It took me a while to figure out why configuration changes weren't always
taking effect after a restart, but eventually I noticed that not all of
the apache2 processes were stopping when using the init script to stop or
restart. A "killall apache2" is usually needed. This was not necessary with
apache2-mpm-prefork.

(On starting there's also an error message saying document roots don't exist,
but the server still works fine. I'm not sure if that's related - I'm using
mod_chroot.)
