Odd "Child NNNN returned a Fatal error" crashes

Google shows some discussion of this problem here, but no real
resolution. I'm not out of disk space, and the problem is of course
quite troublesome in that my whole web server just shuts down. It
seems to happen when I do a graceful reset:

[Tue Mar 22 07:39:03 2005] [notice] SIGUSR1 received. Doing graceful restart

[Tue Mar 22 07:39:14 2005] [alert] Child 2178 returned a Fatal error...

I'm kind of stumped for the moment... I guess I'll try and strace it
tommorow at about this time:-(
