It looks like one of the Intergraph cards that DEC OEM'ed a while back.

AFAIK, there is not and will not be FOSS support for that card, as its
HW details were and remain proprietary.

A nice PCI Radeon (7500/9100/9250) should work, even in 3D... :-)
