AS1000 PCI network device name

Title: AS1000 PCI network device name

The network card on my AlphaServer 1000 4/266 is a DEC "EtherWORKS Turbo PCI".
It worked with the previous operating system, Windows NT, but won't come up for Debian 2.1.

An SRM "show config" shows no network device under "Bus 0, PCI",  but shows the following EISA devices,
which the manual says are FDDI devices (there's nothing like that on this machine):
Slot 3  Option    Hose 0, Bus 1, EISA
  3        DEFEA  fra0.0.0.1003.0           08-00-2B-B6-90-3B
  5        DEFEA  fra0.0.0.1005.0           08-00-2B-B6-A6-84

Shouldn't I see an EWA0.0.0.*.0 device in the PCI section?

I tried "insmod de4x5.o" and "insmod tulip.o", and both report "device or resource busy".
Then I ran the latest tulip-diag and it reported a variety of errors - here are the ones with exclamation points:
Index #1: Found a Digital Tulip, unknown type adapter at 0x9100.
 * A potential Tulip chip has been found, but it appears to be active.
 * Either shutdown the network, or use the '-f' flag to see all values.
PCI bus error!: Unknown 7
Interrupt sources are pending! CSR5 is ffffffff.
WARNING: The EEPROM is missing or erased!
This interface is missing the EEPROM
This is likely the non-primary interface on a multiport board.

Hope someone on this list can shed some light on this!
It's the last thing preventing me from cranking up our Alpha's.
