I ran dvd+rw-mediainfo on another disc from the spindle and from the disc I
described in the original post:

just to recap the dodgy disc was written to once with cdrecord-dvdpro as a
test, I then blanked the disc, and tried to re-record an image onto the disc,
any other suggestions?
