I got this with an IBM Etherjet (UTP only model) when it wasn't
physically connected to a hub.  I think it must be returned for quite
a number of different errors.

Check in /var/log/messages for something like:

Jun  5 22:52:18 psyche kernel: loading device 'eth0'...
Jun  5 22:52:18 psyche kernel: cs89x0.c:v1.02 11/26/96 Russell Nelson <nelson@crynwr.com>
Jun  5 22:52:18 psyche kernel: eth0: cs8920M rev B found at 0x300 media RJ-45, IRQ 5 00 20 35 73 5a b5

Jun  5 22:53:16 psyche kernel: eth0: no network cable attached to configured media
