^TO != ^To. TO also catches Cc and Bcc. See procmailrc(5):
| If the regular expression contains `^TO' it will  be  substituted by 
| `(^((Original-)?(Resent-)?(To|Cc|Bcc)|(X-Envelope|Apparently(-Resent)?)-To):
| (.*[^a-zA-Z])?)', which should catch all destination specifications.
