Now I have. The following small document works for me when I run either
pdflatex or latex/dvips/ps2pdf on it; all the links in toc and minitoc
are clickable.

Therefore I think that minitoc-hyper.sty is obsolete. It would be very
nice if you, Norman, would try this out using current minitoc.sty and
hyperref.sty from a CTAN mirror, or from newer tetex (e.g. my backports
at http://www.kuesterei.ch/).

Consequently minitoc-hyper.sty, being both unmaintained and unnecessary,
could be removed from CTAN, couldn't it, Heiko?
