warnings about "Failed to insert for source by digest"

Hi Peter,

could you give just a brief description (what about, ignore or attention
needed) of warnings such as

W, [2010-10-12 20:12:17#18500]  WARN -- : [indexrun #12] Failed to insert for source by digest, but no file with that digest exists: fsldata 4.1.6-2: fsldata_4.1.6.orig.tar.gz (5e7105a8d989f2eea462a385b4bc56a90d8d4485)

so I do not dive into possibly wrong conclusions ;)
