The printout on my HP Deskjet 3630 Series, hpcups 3.16.10 occurs in color.

This is a really old upstream issue. It seems that for some users using a different printing backend in cups fixes the issue (that is, switching from a hplip driver to a foomatic one). My random guess is that okular is sending the option in a way that is interpreted only by some printing drivers.

If possible, please, send a new comment to the upstream bug [1], sometimes a little nudge goes a long way.

As an alternative pdf viewer, qpdfview seems to handle printing somewhat better (I'm not sure about gray scale, my printer is black and white, :) ), although it's in general not as feature full as okular.
