You have to add a Closes: tag to your changelog that references the ITP.
The changelog has to represent the changes in Debian, so the only entry you need for the first Debian revision is
something like "Initial release (Closes: #1043448)"
The package is clearly not a native package, so please change the source format to 3.0 (quilt).
Please untag moreinfo when you have provided a new version with the requested changes.
