Part 1: failure to install from CD (similar to bug #602380)

I've failed to do a fresh netinst of Squeeze from a CD on a new desktop.  The
install hangs during "Select and install software".  I looked at console 4
while it was hanging and saw 8-line kernel errors like this ad nauseam:

So it's failing to read the CD.  Other possibly useful info:

  The motherboard is an Intel DH55HC, and the CD/DVD drive is a Toshiba
  Samsung DVD Writer Model TS-H663 (see lspci output below).

  The info file on the CD says
    Debian GNU/Linux 6.0.0 "Squeeze" - Official i386 NETINST Binary-1 20110205-14:34

  With another desktop and CD/DVD drive, I verified the checksums on the CD
  with
    md5sum -c md5sum.txt | grep -v 'OK$'
  I also made a second copy of the CD and failed to install with that.

  On the same hardware, netinst CD installs of 5.0.8 and Wheezy ran to
  completion.

  During the failed 6.0.0 install, the CD was successfully read quite a bit
  before it hung.  I found over 30000 files installed on /dev/sda1 (mounted
  on /target).

Part 2: failure to install from USB flash drive (similar to bug #593013)

I made an 8-GB flash drive bootable as follows:

cat debian-6.0.0-i386-netinst.iso > /dev/sdc
sync

It booted, and the install got through the initial language and keymap
selection.  Then:

  No common CD-ROM was detected.

I also failed to install Wheezy this way, although Wheezy from CD ran to
completion.  The version of Wheezy was:
  Debian GNU/Linux testing "Wheezy" - Official Snapshot i386 NETINST Binary-1 20110215-15:58

I was unable to find a description of the CD/DVD drive under /proc but may
not have known where to look in Wheezy.  I was able to mount /media/cdrom and
read the netinst CD.

Hi,

thank you for submitting installation reports, much appreciated.

I read through all the bugs mentioned here (and I'm sure they were read by 
several people at the time they were submitted) and am closing them now as/if
- they (finally) indicated success and/or
- I know from first hand experience that the functionality is working in 
Wheezy and/or
- they only contained very little information and/or
- they contained user errors and/or
- they have been from a development phase where things were not stable and/or
- they are quite old.

If I've closed a bug incorrectly please do reply (it's easy to reopen and I'll 
do if requested) or just file a new one - thats often better, as the bug log 
will be clearer and shorter and not contain cruft.
