I did an installation test with the speakup floppies today. AFAICT these use 
the text frontend for debconf.

After selecting 'other' in the shortlist, there was this huge list of 
countrynames flying across (and off) the screen, all with a number before the 
name to be used for selection. Even with scrollback, I was unable to see the 
beginning of the list (Netherlands, being 88, was just visible :-)

I think the template for the full countrylist should be split into 2 separate 
questions, at least for text frontend, but IMHO for all frontends.
- one question for 'continent';
- one question for selection of an individual country on this continent.

This should reduce the number of options per screen to manageable proportions. 
And as most installations don't use the full list (but have single country or 
select from shortlist), the extra question should not be a problem. 

The only challenge will be to set the default continent based on the default 
country.
As setting the default in the full list does not work ATM anyway, this could 
be a good moment to fix that as well.
