0000:02:0e.0 0104: 1028:0013 (rev 06)

I guess that this is the controller in question, right? Currently our
hardware database lists pci ID 1028:0013 as supported by the megaraid
module. Why did you need megaraid_mm and megaraid_mbox instead of the
regular megaraid module for this card?

Could you send a copy of /var/log/installer/syslog to this bug report?

AIUI you need to turn on some kernel options that are not enabled by
default on stock kernels for support for large quantities of memory. You
could file a bug report on your kernel image package about that if you
like.
