This is very similar to my machine, though I run with a SCSI disk as the
main disk and an IDE for extra storage.  The CMD646 driver is somewhat
questionable:  I noticed that in 2.4.17 (which I'm still running), the
driver refuses to do DMA to the IDE disk.  There's a comment in the source
(linux/drivers/ide/cmd64x.c) including "Due to massive hardware bugs", so
I'd be careful with it.  Did you have DMA enabled somehow?
