Whacked media, drive, or software?



I haven't seen this problem described in the last few months
of the list archive, or in the gentoo forums, so here goes:

Formats and writes of DVD+RW fail with dvd+rw-tools, 
but CD-RW, and DVD-RW can be written by it. Four different 
+RW disks from the same spindle: 3 fail today, but one
succeeded about a month ago. Is this a media problem, a 
drive problem or software problem? I don't have other DVD
writers to test with so I can't directly test the media in
another drive. I also don't have other +RW media to try right
now.

A significant change from about a month ago was that at some
point a update occurred for dvd+rw-tools. I upgraded the kernel
from 2.6.9-gentoo-r9 to several versions today to see if was
a kernel problem. Behavior hasn't changed with any of the
newer versions.
